If your survey project requires IR approval, please complete and submit this form at least 10 business days prior to the survey's proposed launch date. The information you provide here will be reviewed by a member of the IR Office in accordance with the process outlined on the "Survey Policy and Process" section of F&M IR Website.
If you do not require IR approval for your survey project, but wish to request a random sample of up to 200 individuals, please use the IR Random Sample Request Form.
