Review the information presented in F&M’s Human Subjects Research Policy to determine which approval(s) will be necessary for your survey project. If IR approval is not required but you would like to request a random sample, please complete and submit this form at least 5 business days prior to the survey's proposed launch date. More lead time is appreciated, if possible.
